Albuterol Case Study Solution
Throughout case two, there are many medications mentioned. Some of the medications do not directly pertain to the patient, BB, and are instead prescribed for his brother, SB. However, it is important to go through each and every drug in this case in order to better understand why certain drugs were prescribed and their impact on the clinical outcomes of both BB and his brother SB. Additionally, it is important to remember the patient’s age when evaluating these drugs as some may not be appropriate based age restrictions.
Initially, when BB came to the doctor’s office, the case revealed that SB has been sick recently with a cold and is currently taking Zicam®. Zicam®, which contains the active ingredients Zincum Aceticum and Zincum Gluconicum,

Albuterol, which goes by several brand names, including Proventil®, is a nebulized solution that provides relief to bronchospasms in patients.11 Although this should help with the pulmonary issues associated with RSV, it is not a specific indication for albuterol. Albuterol will open up the airways by acting on the beta 2 receptors, which will cause dilation of the smooth muscles in the lungs and improve the ability to breathe. BB should feel some relief in his lungs in a matter of two to six hours. Contraindications are not specifically listed for a nebulized solution. However, in the inhalable albuterol form, some contraindications include hypersensitivities to the inhaler and milk components in the inhalable albuterol form. Since primarily used in children over the age of two, there is not much data present on the side effects of albuterol in patients the same age as BB. An explanation for why the doctor suggested it is that he believed this would reduce the amount of nasal congestion and other upper respiratory allergies and symptoms.12 The suggested pediatric form of Flonase® is actually fluticasone furoate, which is also known as Veramyst® and is prescription only. The fluticasone component, a corticosteroid, is responsible for creating an anti-inflammatory environment in the mucosal passage. Some common side effects are rash and potential for stunted growth in children if given too high of a dose. However, the minimum age for Veramyst® is two years old and Flonase® has a minimum age requirement of four years of age. The case does not provide any more information on the Flonase®, but again the doctor suggested she buy it over the counter, which would make the minimum age four years old. The decrease in hospitalizations was by 39% by LUM (600mg/day)-IVA (p-value=0.003) and 61% by LUM (400 mg every 12 hrs)-IVA (p-value < 0.001) pooling the TRAFFIC and TRANSPORT studies compared to the placebo.
