Anxiety Disorders Case Study Essay
William Rich is a 29 years old, homeless male and Afghanistan War Veteran. He appeared psychotic and unable to communicate. In 2012 he lost his right leg when a mine exploded and he uses a wheelchair. William stated he doesn’t want to stop using drugs (crystal and spice) because it relieves his symptoms of depression and anxiety. William shared that when he was a teenager he was placed on a 51/50 hold when he swallowed a bottle of pills after a breakup with his girlfriend. He stated he had a physical altercation with his father and was burdened by remorse, guilt and shame. William shared that he felt hopeless about life and could not endure another day on the street. William suffered from weight loss, poor hygiene, lack of concentration, and exhibits symptoms of; memory loss, anxiety, dizziness, and experiences nightmares and horrible memories. William also feels confused, agitated and full of frustration. Problem: William’s problems are the following; PTSD, with anxiety disorder, major depressive disorders, stimulant use disorder, unspecified cannabis-related disorder, other specified disruptive impulse-control and conduct disorder, and unspecified …show more content…
As a provider, the goal would be to encourage William to reach sobriety so that he can be reconnected with his VA benefits. In doing this we would suggest William to enroll in a harm-reduction (HR) program that’s VA connected, or community based, and monitor his progress to sobriety. In addition, doing Cognitive Behavioral Therapy (CBT) with William to improve his way of thinking and reminding him that there’s still hope. William’s use of drugs and feeling dysphoric is what ultimately caused him to sever ties with his family, wellbeing and benefits. By treating those problem areas with CBT and harm reduction William can rebuild those broken ties and live a meaningful

    Unfortunately, there is a commonality that each treatment provider is faced with a client that only has one single issue. However, in this example, the client has more than one issue ranging from substance abuse to mental illness to homelessness. The provider will then need to determine if the client will need additional assistance with other issues that may be outside our scope of practice. For example, if the provider is working in a substance abuse treatment program the primary focus is helping the client to stop using drugs and/ or alcohol. As the client is no longer supposed to use substances or drink, the client can no longer reside in an environment where substances and/or alcohol are being used; this can become a trigger for the client and risk the client’s sobriety and recovery. This client is now homeless and will need assistance with housing. As the client continues in sobriety the treatment provider may notice that the client appears to be struggling with mental health issues that have not been previously diagnosed; this client will now need to be referred to a mental health provider for further evaluation and possible medication management.
