MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:
This Memorandum of Agreement, entered into by and between:
LYCEUM OF THE PHILIPPINES UNIVERSITY – COLLEGE OF BUSINESS ADMINISTRATION, an educational Institution duly organized and operating under and by virtue of the laws of the Philippines; with offices, facilities, and campus located at Governor's Drive General Trias, Cavite, represented in this agreement by the Associate Dean of College of Business Administration, Reynaldo C. San Mateo, hereinafter referred to as the “FIRST PARTY”;
– and –
(Name of the Company) with principal office located at (address of the company), represented in this act by its, Practicum Supervisor, (Name of Practicum Supervisor) hereinafter referred to as the “SECOND PARTY”.
WITNESSETH:
`WHEREAS, the FIRST PARTY offers academic program in (course). As components of its curriculum, students are required to undergo practical training by attending work-related activities in offices of government entities, business establishment, or corporation engaged in business, services, calling profession, activity, or , operation related to of has connection with the students chosen field of study;
WHEREAS, the SECOND PARTY is in need of on-the-job trainees who will assist and support its officers and employees in their daily tasks, and the FIRST PARTY has students who are qualified and willing to perform such task;
WHEREAS, the parties herein have agreed that the FIRST PARTY shall provide the SECOND PARTY with on the job-trainees, (hereinafter referred to as the TRAINEES) who will assist the SECOND PARTY in its daily operation;
NOW, THEREFORE, let it be remembered, that the parties hereto have hereunto agreed on the following:
1. This agreement shall be valid and binding upon the parties for a period of one (1) year, i.e. from April 2014 to March 2015. Either party may pre-terminate this agreement by a written notice to the other party at least thirty (30) days prior to the intended
