Reflective Account

There was a time when I went to administer the night medication to customer X and realised that he was heavily under the influence of alcohol. Me and my colleagues had noticed that he had been consuming brandy all day. In addition staff had been seen purchasing 2 bottles of brandy by one of my colleagues.
Customer X was taking the following medication which was recorded in the MAR chart:

Citalopram 20mg tablets – one tablet daily (morning)
Citalopram 10mg tablets – one tablet daily (morning)

Chlorpromazine 50mg tablets – one tablet 3 times daily (morning, midday, night)
Chlorpromazine 100mg tablets – one tablet 3 times daily (morning, midday, night)

Valproate Semi sodium 500mg tablets – one tablet 3 times daily (morning, midday, night)

Sodium Chloride MR 600mg tablets – 2 tablets 4 times daily (morning, midday, teatime, night)

Promethazine HCI 25mg tablets – one tablet daily (night)

Procyclidine 5mg tablets – one tablet daily (morning)

Lorazepam 1mg tablets – one twice daily (morning, night)

Omeprazole 10mg capsules – one capsule daily.

Because it was out of hours, I contacted the crisis team to inform them of the situation and to seek advice. I was advised not to give any medication to Customer X as mixing it with alcohol could have an adverse reaction and be life threatening.
I then contacted the on-call manager for my workplace to inform of the occurrence.

I tried to customer X the reason why he would not be given his medication but he seemed to have very reduced concentration due to being intoxicated with alcohol. I waited for the next day when he was sober and informed him of all the actions taken the night before regarding his medication. I also gave him information and advice about the risks of mixing any kind of medication with alcohol and offered support and information about alcohol support

    One day I was ask to transfer Mrs X from bed into wheelchair. Mrs X is paralised so to transfer her a full body hoist is needed. I had to call for help, it wouldn’t be safe to do this by myself. So while I was waiting for the other carer to arrive I have explained to Mrs X what and how we are going to do it. I have also cheked the hoist and battery if its fully charged and operational. I put right size sling on Mrs X with her cooperation. I carefully manovered hoist close to bed, then I hooked sling onto the hoist with short hooks on to of the body and long hooks on bottom. That was when my collegue arrived to help me with transfer.We asked Mrs X if she is ready when she said yes by pushing button on hoist remote I raised Mrs X up, while my collegue was holding her to stop swinging. When she was up I reversed away from bed. When I was in positionfor lowering Mrs X I have opened legs of the hoist. When my collegue manouvered wheelchair into position I was supporting Mrs X on back and knees. When wheelchair was in position my collegue took over of supporting Mrs X and I asked both Mrs X and my collegue if they are ready to lower Mrs X into a wheelchair. After positive answer I have lowered Mrs X into a Wheelchair and after that me and my collegue unhooked sling from the hoist and I safely moved the hoist away.     I work in a Jewish care home on EMI dementia nursing unit for the elderly. My clients suffer from dementia. They are in different stages of that illness, from mild to severe. Some of them are in end of live care. My clients come from different backgrounds then me and where raised in a very different way to how I was raised. It is important that I will remember this and respect their beliefs, as we all have personal beliefs and preferences based on our background and upbringing. I have been brought up in a catholic society where religious education was compulsory, however we still studied various other religions and were taught to respect all people and there beliefs. Prayers were a daily thing at the end of each assembly and whether or not I wanted to pay attention to the prayer time I had to respect the people that did. Religion may affect people in many different ways, mainly because we all come from different backgrounds and are taught differently. However, it is important to respect how people honor their religion. I am an catholic, however I still make time to talk to my clients about their religion and sometimes have prayer time with them, and when possible I take the clients to synagogue. I spend my time with them on Saturdays (in Jewish culture Sabbath is a day of rest) even though my own beliefs teach me something different, I know it makes the client happy so it makes me happy to do this. I know it is something that they feel so strongly about and it is amazing at how much I really now look forward to going to synagogue with them. It is part of the job that I do and parts of showing the clients that we care about what makes them happy and that we respect their beliefs even if ours are different.…
