Bi-Weekly ILP Review
On 12/2/2015, CM met with the client and to complete Bi-Weekly ILP Review. Client was dressed in brown jacket, black slack pants, red sweater, black scarf and baseball hat. In the meeting client looked paranoia. She didn't made eye contact. She constantly looked over her shoulder, she walk with book bag on her shoulder and two shopping bags. Client affect was inappropriate.

EMPLOYMENT UPDATE: Client is unemployed and she report she is actively searching for employment and submitted employment search log. CM mentioned to the client for the next bi-weekly ILP Review for the client to bring the following: Resume and Cover Letter and for the client to participate in the onsite Resume Workshop. CM will also assist the client in applying for

SAVING UPDATE: Client stated that she currently has no savings.

SUBSTANCE ABUSE UPDATE: Client denies any substance abuse issue

MENTAL HEALTH UPDATE: On 11/16/2015, Client met with the onsite psychiatrist and she was diagnosed with Axis 1: PSTD (Post-traumatic stress disorder) - F43.10 (Primary), rule out. CM will refer client to mental health counseling for individual counseling to address her PTSD.
HOUSING UPDATE: Client exit strategy date 6/30/2015. Client is GP and housing package completed and circulating. Client is also advise to seek and secure employment to be able to qualify for the LINC V voucher.

REVIEW ILP AND CLIENT RIGHTS ANDCLIENT CODE OF CONDUCT:
Client is non complaint in face to face and seeking and securing employment.
CM reiterates to the client she must accept the first suitable housing offers. Client must also accept first suitable jobs offer. Client must adhere to the shelter 10pm curfew.

: Client sometimes missed the scheduled face to face meetings. CM reiterates the shelter rule and WORKER ASSESSMENT: Client appears to have the ability to identify and complete goals in a timely fashion. Client must attend to all schedule meetings, client must accept the first suitable housing offer, client must bring a copy of the employment search log, and client must participate in all scheduled meetings. Client must participate in mental health counseling. Client must meet with CM every Wednesday; Client must maintain dorm clean and clutter

    The PCL has good psychometric properties, including internal and test-retest reliability and convergent validity (Screening tools help in assessing trauma, 2011). The PCL contains 17 questions that map onto the three DSM-IV PTSD symptom clusters: re-experiencing, avoidance, and arousal (Screening tools help in assessing trauma, 2011).
