COMMUNITY ASSESSMENT AND ANALYSIS OF PHOENIX Community assessment and analysis by the community health nurse is of vital importance. This means understanding the community at large and in details on health matters and issues affecting health as well as the healthcare system used in the community. This is done through data collection and analysis, identifying community assets, resources, barriers to health and health needs in the community. The community health nurse then disseminates information on how to improve the community health. The writers used functional health patterns to assess and evaluate the community and the nursing process to come up with a community diagnosis that all led to recommendations. The city of Phoenix is located in the state of Arizona. It is the sixth largest state in the United States with a population of approximately 1.4 million (City of Phoenix, 2013). Usually considered flat and desolate, the city actually spans over 500 square miles of rich terrain and is bordered by mountain ranges to the north, south, east, and west. Immediately beyond the city’s boundaries are the neighboring cities of Cave Creek, Glendale, Scottsdale, Mesa, and Chandler. Because of the close proximity of neighboring towns they are often referred to as part of the Phoenix Metro area. There is a tendency for overlap of community services, which includes public transportation, collaboration of health care facilities, and cooperation between law enforcement agencies. The people of Phoenix, also referred to as Phoenicians, are known to be friendly and accommodating. People who move to Phoenix come from all over the country for the many amenities, vast sunshine, and family oriented lifestyle. Some population groups that are living within the Phoenix borders include retirees, baby boomers,
