The model proposed for chronic rehabilitation services at the new rehabilitation unit at Memorial Hospital will be based on minimum staffing patterns for hospitals providing chronic inpatient treatment. The model will be driven by the desire to provide quality care despite the staffing constraints. There are several reasons for selecting this model for the rehab unit. First, this model builds on the premise that staffing should be based on the objectives, mission, and vision, nature and severity of illnesses, and the extent of impairment (Chinn & Maeona, 2010). The model also focuses on the program and the balancing of the factors that support the interactions among the administrative staff. The model also emphasizes on nurse qualification, consistency of a unit with hospital administration. organization and services delivery, pertaining to psychology, education, social work, pediatric medicine and occupational therapy should be done in a professional and ethical manner by qualified personnel (Chinn & Maeona, 2010). The model also places unit and program roles and responsibilities and service quality on the administration.
The staff at the inpatient rehabilitation unit will specifically seek to assist patients suffering from injuries or diseases that have made them experience loss of function.
The model’s other key objective will be helping patients regain their independence while undertaking their routine and daily living activities. Also, the staffing-based nursing care delivery model seeks to ensure that patients return home and re-enter the community as active, productive, and independent people (Chinn & Maeona, 2010). The model will focus on the main experiences of chronic rehab patients, which include but are not limited to blighted movement, body balance and coordination, impaired strength and reduced movement options, and loss of sensory ad visual abilities. Also to be addressed by the model is chronic rehab patients’ experiences of cognitive impairment, speech or communication impairment, and swallowing
problems.
The Rehab Unit Team
The model envisages a multi- and interdisciplinary team to help succeed in patient rehabilitation. The first requirement placed upon the team is complete dedication to the objective of the unit, which is meeting and exceeding patients’ expectations regarding their physiological, physical, social, and mental wellbeing. The team must be driven by the desire to hasten patient wellness, fitness, confidence, and independence, especially pertaining to their ability and capacity to conduct their daily lives as normally (Schenkman et al., 2011). To achieve these objectives, the RNs and LVNs/LPNs are to work towards giving patients the right information that would promote their transition from hospital surrounding to home and community environments.
Daily/Routine Activities
The achievement of the unit’s objectives heavily relies on the daily activities in which patients will engage. Besides the basic activities such as physical, occupational, and speech therapy, chronic patients at the unit will undertake activities such as grooming, dressing by themselves, engaging in leisure activities, and taking meals together. All these activities will be aimed at promoting patients’ independence, normal functioning, and socialization. Families, friends, and colleagues will also be permitted to visit patients at the unit. Also, patients’ loved ones will be allowed to take part in patients’ treatment, recovery, and educational activities to help in the transition from hospital to home or community environments.
Nursing Staffing Pattern
Though a tiring process, a care-hour staffing pattern based on the workforce and time constraints was developed using the following parameters;
Estimating a core staff per shift
Bed number in surgical unit= 7 bed
The average daily census for 12 week = 6 patients
The typical daily care hour to be provided= 4 hour per /24 hour, implying total hours of care needed= 6x4= 24 hours.
If the work day is 8 hours, then 24 divided by 8 means 3 staffs are required in the unit in 24 hours.
Total of 3 staffs X 7 days / week = 21 shift / work
If the employee work 4 hours shift / week, then for 21 shift/work,
4 = 5.25=5 workers required per shift
The staff required on each shift, for each day and week can be based on day, evening, and night shifts on 45%, 35%, and 25% respectively, depending on patient population.
