Ms. Ferguson-Campbell is a successful entrepreneur for over 25 years. Ms. Bernice Ferguson professional resume is well diverse with 10+ years as a Registered Nurse (RN), 20 years as chairperson of the New Jersey Department of Health and 15 years of successful real estate trading. Kathy Ferguson whom is the director of the facility executes daily operations of the daycare. She holds a degree in early-childhood education. 9:00-9:47AM. SC received an email from OLTL noting approval for increase in PAS hours. SC made required changes in SAMS and Oracle. SC contacted the provider Home Health Care on 12/31/2015 at 9:19 and spoke with Ferline an after hour’s receptionist. SC left detail message stating reason for calling and contact information. Ferline states that the office will be opened at 9:30 and she forward SC message. SC thanks Ferline and end call. SC place call to Pa caregiver Caroline and informed her of service approval. Caroline was pleased to hear stated that she Pa’s current DCW ( Ruddy) is no longer available to provide care to Pa in the same manner he was able before and she is in search for a replacement. SC suggest Caroline use an agency aide until she finds a suitable replacement because Pa’s needs are high and bot having adequate help can leave her burned out and Pa at risk greater declined.…

CCIB received CPS referral #0972-4764-8677-3023697 from reporting party (RP) stating that on 5/15/17, foster child (FC) Destiny Salazar (age 18, DOB: 04/30/99) contacted RP to report that she was kicked out of the foster home by Foster mother (FM) for getting home late the prior evening. FM would not allow FC to return to the home to pick up her things. FC was staying with her boyfriend. RP contacted FC to obtain her verson of events. FM stated that on a previous occasion when FC was late, she sat the child down and informed her that if she were late next time, FM would kick her out. FM advised RP that "a 7 day notice is a courtesy and I don't have togive a 7 day notice." FM also stated that previously, she would contact the social worker…
The reporting party (RP) is the mother of resident Cody Stelling DOB: 10/20/95. Last week (exact date unknown) at a meeting with resident, his mother, service coordinator, and facility administration Cody was issued a 30 day eviction notice. The RP stated Cody was admitted three weeks prior to his eviction notice. According to the RP the facility was evicting the resident for issues discussed for his needs and services plan prior to admission. The RP stated the facility removed an adaptor from Cody to prevent his playing video games and was not returned. A framed poster of a Lamborghini was removed by staff and not returned. The resident provided a list of missing items to caregiver Juana upon leaving the facility. Those items have not been returned.…
