INTERVIEW SETTING
On 3/1/17 I met Mr. Kobetic at the office of vascular surgeon Dr. Levin. Mr. Kobetic was in a wheel chair. He reported he got up late and did not have time to put his prosthetic leg on. He was alert and oriented and willing to work with a nurse case manager.
MEDICAL FACTORS
Mr. Kobetic was released from the hospital from an unrelated admission. He was driving his car; he reports he is not sure what exactly happened. He said he had a vacuum device on the left foot from a recent surgery and amputation to the left foot. He is not sure but felt he had the tubing tangle on the brake causing him to not be able to stop. After the accident he was admitted back into the hospital. CURRENT STATUS
Mr. Kobetic reports he has some pain in his back and ribs. His pain medication is making that tolerable. The left leg stump has one small area that is open, but otherwise very healed. Mr. Kobetic is very independent with his personal care.
CURRENT TREATMENT PLAN
I spoke with Dr. Levin and obtained the history of the original amputation of the toes. This was due to an infection that has been ongoing since August 2016. I asked if the auto accident had any impact on the knee for the below the knee amputation. Dr. Levin said that the recent amputation is not related to the auto accident but is related to the infection and would have occurred in spite of the accident.
