On Thursday December 31, 2015 at approximately 1949 hours, FHEO Security Officers were dispatched to room #503 Beds 1 and 2 for two Disorderly Male Baker Act Patients who were acting up and causing problems for their assigned Psych Sitter, including one patient from Bed 1 who kept trying to leave the room. Security Officers Omar Alonso, Carlos Ayuso, and James Johnson arrived and met with Nurse Celia Baggya and Psych Sitter Andell Philip while Security Supervisor Steven Evans arrived later and met with Charge Nurse Charlene McGee. Nurse Baggya and Sitter Philip both stated that the patient in Bed #1, Benjamin Atsu (DOB: 01/23/1991; FIN# 85028628) became aggressive towards medical staff and was also trying to provoke the patient in Bed#2 who was Armando Montalvo (DOB: 08/15/1986; FIN #85028608). 3 year old Tina Gerhardt was admitted to the abulatroy surgery unit at Nightingale Community Hospital by her mother for bilateral myringotomies (insertion of ear tubes). On Saturday, February 6, 2016 at approximately 0016 hours, FHEO Security Officers responded to a call in regards of a Disorderly Patient (53D) who refused to come out the restroom at the MRI area. Security Officers Brandon Rodriguez, Omar Alonso, and Steven Evans arrived to find a combative patient inside the restroom and refused to come out. MRI Tech, Noreen Dailey stated that the patient had been in the restroom for about 20 minutes, at which time when Noreen finally open the door, found the patient trying to pull up the sink off and acting in a strange manner like cleaning herself over and over again. RA would not talk with Treva. RA constantly asked Treva to leave. RA’s daughter & family kept talking to the RA about getting services in the home, especially wound care. Family provided information that the RA will have a physician to come to the home to treat the RA’s bedsores, a nurse to monitor the RA’s meds, and an aide for the RA’s personal care. In report #17-067-00223 the RA left AMA from Hidden Lake Nursing Home. The concern was that his blood sugar was extremely elevated, and he left without taking his oxygen. Treva made a home visit to interview the RA. RA would only talk for a few minutes, but he kept telling Treva that he was fine, and he did not need any assistance from DHSS. Treva was able to explain to the RA that the concern was that he would go into a diabetic coma, but the RA ended the conversation by closing the door. Treva left her card at the door along with resources on diabetes. Intoxicated male patient, Adam Barger (DOB:02/05/1977 FIN #86198457), came in through the Emergency Department on Sunday 9/18/2016 at approximately 2300 hours, he ended up in the Specialty Care Unit (SCU) because he was highly intoxicated, from there he was taken to ICU on Monday 9/19/2016 and was finally transferred to 2 West Unit, room #2105 on 9/22/16 sometime in the afternoon. Facts: On July 4, 1972, Mrs. Mitchell started working at the Center in Lovington as a nurse’s aid. After being there for a year, she had additional duties at to her work load. She now served as a relief medications nurse two days out of the week. Mrs. Mitchell was starting to act out. She had an argument with the head nurse, Mrs. Stroope, in a crowded area of the Center. There was an incident where Mrs. Mitchell came to work out of uniform. On that particular day the Federal Regulation Inspectors were there for a visit. She was told to go home and change, but she refused to do so. The next day she came back into work out of uniform. She was told to go home and change, but this time she did as she was told. The reporting party (RP) disclosed resident Forrest Thomas age 18 has a developmental disability, tumors on his brain, and a heart condition. According to the RP the resident has a mental function of a 13-15 year old. The resident disclosed that a staff member (name unknown) has made inappropriate sexual comments including asking the resident to take a shower with him, asking the resident to masturbate in front of him, showing the resident an apple and asking if that what the resident's penis looks like. AB is a 17 year-old, heterosexual, African American male. AB was transferred to North Texas State Hospital, Vernon campus (NTSH-V) after being found to be Manifestly Dangerous by an Institutional Review Board at Terrell State Hospital (TSH) on 3/28/14. While he was TSH, his behavior deteriorated. He had multiple incidents ranging from verbal threats to physical aggression against peers and staff. The physical aggression was described as shoving, pushing and punching peers. AB engaged in property destruction, e.g., throwing chairs, tables, breaking furniture (e.g., metal lockers) and using the broken pieces to attack staff. AB was transferred back to NTSH on 3/28/2014 where he remains. AB has a medical diagnosis of epilepsy and takes Clonidine daily to control his seizures. His last seizure was about 2 years ago.…
On Thursday September 14th at around 12:30 pm the hospital had a sentinel event which involved a missing child. The event details are as follows: Tina was scheduled for same day surgery and was brought to the hospital by her Mother. Her Mother was informed that the surgery would take approximately 45 minutes and that Tina would be in recovery at least 1 hour. Tina’s Mother informed the pre-op nurse that she had an errand to run involving Tina’s older sibling. Tina’s mother left the pre-op nurse her cell phone number with instructions to call her if Tina got out of surgery and was ready to go before she returned. Prior to Tina’s mother returning Tina’s father showed up to check on her, and finding that she was ready to go he took her to his house until the mother could pick her up. When Tina’s mother returned 2 ½ hours later she found Tina missing and the staff did not know that her father took her. The hospital activated a Code Pink and notified the local police department. When the hospital security interview the mother they learned the parents were divorced and that she had full custody of Tina and her other siblings. The local police found Tina at her father’s home within 30 minutes of Tina’s mother returning to the hospital. No charges were filed against the father.…
