On April 12, 2013 Mr. Purgert visited with his Nephrologist Dr. Steven Mischel, D.O because of a Nephrology consult which was requested ( who requested it?) to help manage his renal transplant. Dr. Michel noted that he had end stage renal disease secondary to polycystic kidney disease, Renal transplant in 1992 with a recent failure requiring dialysis, Severe hypertension at times, Coronary artery disease, and a Successful renal transplant on January 2012. Dr. Mischel recommended an angiography with stenting, insulin for his diabetes. …show more content…
Mischel wrote an attending physician statement for Mr. Purgert. He noted that he had chronic fatigue, chest pain, dypnea diabetes and history of a renal transplant. Dr. Mischel noted that Mr. Purgert is not allowed to do any heavy lifting and he cannot go to work even if accommodations were made for him in the workplace. However if he does decide to go to work he can only work less than 20 hours a week. He also noted that at home he can perform mild exercises. Dr. Mischel reiterated these symptoms in an attending physician statement on May 31, 2013, he also stressed that Mr. Purgert is still unable to return to
