Chart Audit Case Study
Hi,

During a chart audit I found several deficiencies with Prentice Bibbs this will be his final written warning. I have enclosed the discipline for your review.

We are also having issues with him not completing his assignments on time. He also has been reporting many Therapists not available with in the month.

He continues to struggle with time management I have counsel him in the past as well as reeducated and shadowed him to help give him tips on improvement. This continues to be an ongoing issue. I have forward you the emails in regards to time effectiveness as well.
