PATHOPHYSIOLOGY OF CHRONIC OSTEOMYELITIS

Red text - signs & symptoms manifested by the client. * - as evidenced by the biopsy result

Analysis: The client was diagnosed with chronic osteomyelitis on her 2nd confinement after she sought consult for a tender mass growth on her Right radius. Chronic osteomyelitis is an infection of the bone usually caused by extension of soft tissue infection, direct bone contamination from bone surgery or an open fracture or traumatic injury and hematogenous spread from other sites of infection. In the client’s case, it can possibly be traced to her history of fall approximately one year

    Deep to this area of apparent ulceration there is bright T2-signal present at the dorsalateral aspect of the foot at the level of the mid to distal metatarsals suggesting edema and and/or inflammation in the dorsal soft tissues in that region. I do not see definite replacement of the normal bright marrow signal on the T1-sequence within the tarsals or metatarsals. Therefore, I do not see definite osteomyelitis. Certainly the findings are highly suggestive of a rampant cellulitis. Unfortunately, I have no plain films available for correlation at this time.     HISTORY OF PRESENT ILLNESS: The patient is a very pleasant 57-year old female, a native of Cuba, being seen for evaluation and treatment for sores in her mouth that she has had for the last 10-12 days.
