Background/Problem:
Baylor Scott and White Medical Center – Hillcrest (BSWMC-H) utilizes the professional practice model (PPM) to evaluate the quality of care provided to ensure existing processes maintain safe passage for patients, through the continuum of care.
Frequently, clinical nurses evaluate the PPM during daily huddles on the units.
This evaluation helps determine if the PPM continues to be relevant and representative of the current nursing practice. During one such discussion on 5 South, the clinical nurses discovered an issue in the discharge process related to the PPM 3A’s of authority, autonomy, and accountability. o The discharge checklist was implemented. o The discharge checklist is initiated on admission and completed by the clinical nurse, case management, social work or physical therapy throughout the patient’s stay. o Prior to discharge, the primary nurse or nurse who is discharging the patient reviews the checklist for completion and signs the checklist when completed.
Because of continuous evaluation of the PPM with clinical nurse involvement, improvements were made in patient satisfaction with the discharge process. Implementation of the checklist has resulted in patients being more satisfied with their instructions for care at home and has resulted in safer passage for patients through the continuum of care, addressing the issues identified through evaluation of the professional practice model. In the 1st quarter of 2015, the 5-south patient satisfaction with Instructions for care at home score was 88.3% always. In the months that followed implementation, the 5-south team increased their scores to 90.9% in quarter 4, 2015, 90.9% in quarter 1, 2016, 89.5% quarter 2, 2016, and 90.4% quarter 3,
