Clinical Pharmacy Field Experience Essay
I work as pharmacist in a community pharmacy in Quorn, a small town of South Australia. Our pharmacy not only provide services to community patients but also to only hospital and community home in the town. I need to perform various jobs, like dispensing medications, communicating with the prescriber and other health professionals like nurses, carers etc.

The following constitute an important part of my job
· Review medication list to prevent errors, disease state management, improve patient awareness of the treatment, monitor treatment responses, check and improve patient compliance
· Communicate any medication errors to the prescriber for making the relevant change[s].
· Advice regarding the safe, appropriate and cost effective use of medications.

I would like to enrich my knowledge by studying master of clinical pharmacy. This course is designed to enable students to establish skills in evidence-based practice, quality use of medicine, practice-based research, and provision of clinical pharmacy services at a high standard.

I would be better equipped to perform the following roles by studying the course on clinical pharmacy.

· Provide integrated care assuring continuity of information on risks and benefits of drug therapy.
· Improve pharmaceutical care of patients.
· Pharmacist initiated drug review to reduce the number of prescribed medicines and frequency of drug related problem.
· Influence prescribing by doctors because of appropriate knowledge about therapeutics.
· Provide medication therapy evaluation and recommendation to patients and other health care professionals.
· Advice regarding the safe, appropriate and cost effective use of medications.
· Review medication list to prevent errors, disease state
