FUNCTION OF COMMUNITY HEALTH TEAMS (CHTS)
Health teams guarantee that every family in the community is periodically visited and attended by a health provider and other community workers

Do house to house visits
Reach all families with priority for CCT/NHTS families and 12 breakthrough areas with highest concentration of NHTS families
