Complete Heart Block Case Study
ACKNOWLEDGMENT

It gives me an immense pleasure in presenting this case study, which is made for the partial fulfillment of the requirement for the fourth year of B.Sc. Nursing Programme. We were provided 8 weeks of the clinical practice, at Shahid Gangalal National Heart Center. During our 8 weeks of clinical exposure each student had to do a detailed study of one cardiac problem. During my clinical exposure to Coronary Care Unit (CCU) for 1 week I got an opportunity to take case for study. We had morning, evening and night duty for complete exposure to clinical picture.

Regarding this process, I would like to express the heartfelt gratitude to various personnel who helped in different ways for the successful accomplishment of this work. This has been possible with the tremendous support, encouragement and patience from them.

Firstly, I would like to give cordial thank to our Nursing Director Miss Nani Maiya Shrestha, our Co-ordinator Madam Sabita Poudel. I am equally thankful to our Supervisor Madam Narayani Paudel for providing full support, ideas and inspiration for presentation of this case study.

I am indebted to all the faculty members, teachers, staffs of entire Coronary Care Unit for their mutual co-ordination which aided in making this case study .Last but not least, I would like to continue my thanks to the patient and patient party for providing the fact necessary information without which this case study would not have been possible.

Lastly, I would like to thank all the people who have direct and indirect link with me to make this case study successful.

The clinical skill that I have decided to reflect on in this essay is the manual blood pressure test that I have performed on one of my colleague during a simulative clinical skills session. The aim of this essay is to reflect on this experience, so as to learn from it, identify areas of concern and increase competence in this skill accordingly. To start the essay it is the custom to at least give a definition of blood pressure test, and understand how to interprete the results. I will then proceed to reflect on this whole experience. I will continue by revealing two issues that I have encountered while performing the blood pressure test, and exposed approaches to minimise their reoccurrence. Interpersonal skills will be demonstrated by showing compassion and understanding during the whole process, so as to ensure the “patient’s” comfort and meet all his personal and healthcare needs. To conclude, I will go over the main points of the knowledge and skills that I have gained during this exercise, and explore ways to continually improve and use them during my future practical placement opportunities.…

I was very fortunate to get hands-on clinical experience here in Internal Medicine and Cardiology for four weeks. Apart from history taking with examination in both outpatient and inpatient settings with the case presentation in morning rounds and regular daily follow-up of patients, I had the chance to observe sleep studies in detail, how we practically look for any abnormality in each stage of sleep really fascinated me. Most exciting part of this rotation was getting familiarized with electronic medical records, which took me some time to adjust, but I was quite comfortable in it by the end of my second week here. Owing to my interest in Cardiology I gave a presentation on valvular heart diseases and words of encouragement and praise really…

These hypotheses include: a decreasing rate of angina, declining heart disease risk factors, changing provider practices (troponin testing, increased used of revascularization, changes in diagnostic testing), increased use of the ED and outpatient clinics to handle acute chest pain, and changing ICD-9 CM coding. The hypothesis that the decline was due to angina being moved from primary diagnosis when a patient is diagnosed with another cardiac complication was argued due to the finding that angina being recorded as any diagnosis also declined from…

(CDC, AHA). This disease has been established as the leading cause of death in the United States (AHA). One in every five deaths has been associated with CAD in the United States. The National Heart, Lung, and Blood Institute states that “more than a million persons in the U.S. have a heart attack and about half (515,000) of them die in each year. About one-half of those who die do so within 1 hour of the start of symptoms and before reaching the hospital”. A heart attack happens to a person when the blood flow and oxygen supply to heart muscle is blocked, and it is mostly caused by the Coronary Artery Disease (CAD). CAD occurs when the arteries that supply blood to the heart muscle (coronary arteries) become hardened and narrowed. It often causes irregular heart beat or rhythm by blocking blood stream. The National Heart, Lung, and Blood Institute suggest that “everyone should know the warning signs of a heart attack and how to get emergency help”.…
