Rogers states that concepts are more complicated to comprehend so they need to be refined continually and introduce variations so that they are more clearer and bring out more useful meaning. So, concept analysis helps researchers to reevaluate the concepts which has been out there and bring improvement on them as nursing is becoming technologically advanced.
Method of Analysis Everyone who goes to the hospital expect to receive good and quality nursing care. Nursing care has been there since the time of Florence Nightingale but If you ask yourself, is it the same nursing care which was then and now? Absolutely, not. Nursing care being given now is more advanced than then but the compassionate nursing care remains the same. Reviewing the article done by Kellogg Victoria on quality nursing care: concept analysis, which it`s main purpose is to analyze the nursing care given as the health care costs are going up, increasing technology, people are living longer and just the financial resources are going down. So, the stakeholders are looking for ways to improve the nursing care of patients. This article was looking at ways to clarify the concept of quality nursing care. How can a layman understand what quality nursing care is all about? The aim of this analysis was to make sure the concept was clear, to identify substitute or related concepts and to recognize the criteria used. Rogers` evolutionary approach was used to analyze the concept. Use of computerized database was utilized in which the key words employed were “quality” and “quality nursing care”. According to McEwen & Wills (2014), the evolutionary method of concept development is a concurrent task approach. Which means the tasks can be done at the same time instead of going step by step in a sequential manner
Steps of the Process According to McEwen & Wills (2014), the following are the Rogers’ steps in process of concept analysis used by Kellogg:
1. Identify the concept and associated terms. In this case the concept is quality nursing care.
2. Select an appropriate realm (a setting or a sample) for data collection. The setting was the place of work which is Pennsylvania University. Not exact place is mentioned.
3. Collect data to identify the attributes of the concept and the contextual basis of the concept (i.e., interdisciplinary, social cultural, and temporal variations). The use of CINAHL, Health Star and Dissertation abstracts. This was all computerized databases.
4. Analyze the data regarding the characteristics of the concept. The data collected was analyzed in full by Kellogg.
5. Identify an exemplar of the concept, if appropriate. In this case no exemplar was identified.
6. Identify hypotheses and implications for further development. The hypotheses are that, after going through all these steps, Kellogg concluded that the overall concept of quality nursing care is an intricate matter with the patient and nurse coming together to interact within the process, structure and outcome framework. Further research to be done to clarify and explore the roles of the patient and the nurse. Therefore, improving quality nursing care in hospitals is required as nurses are the only health care professionals who are with the patients 24 hours.
The Concept to a Practice Situation The concept of quality nursing care is really a complicated issue to apply as it depends on what kind of patients you have or receive.
Some patients like alcoholics are very hard to deal with because most of them come to the units withdrawing. But as a nurse, you have to do the best you can. Some patients come to the hospital with hidden agendas. For example, one time few years ago, I received a patient from emergency room whom I was told he was a very difficult patient to deal with. I was kind of prepared when he came in. I was very pleasant and tried as much as possible to make him comfortable. I practically did everything he asked me to do. But guess what? He still complained and started threatening to sue the hospital for I did not know what. I let him calm down and I talked to him firmly. I told him that whatever agenda you have before you came here and start threatening us, does not work that way. You will just fight a losing battle. Everything you are doing is being documented and recorded, so please just stop. After that, he never threatened anybody and he really appreciated after he left. So, Kellogg was right when she mentioned that giving quality nursing care is complicated because it is between the patient and the nurse. But as nurses, striving to give the best care to the patients is what we desire and it is our focus as we provide
care. Conclusion Concept analysis is always the way to improve our practice as it helps us nurses improve on the concepts or theories that are already out there. More research on the concepts will improve or make nursing profession be an outstanding profession. As Kellogg did a concept analysis on quality nursing care, she made it clearer about the relationship between the nurse and the patient as complicated as it depends on an individual. That is why individualized care is very important. Always to put in our mind that you cannot please everyone matter how better a nurse you are.
