However, CPN must establish the safety boundaries and inform SU, crisis plan should aim to capture a summary of risk identified, formulations of the situations which risks occur, including warning signs, and strategies to be taken by CPN and physicians.
Enable effective communication and the safety management plan must be clearly documented. Unambiguous reporting and documentation is important for continuity of care and communication with team and SU. Note changes in the level of risk or protective factors for ongoing review by CPN and multidisciplinary
