Introduction
The aim of the project is to look at the specific needs of the Older Person who has the chronic condition of Dementia. I will describe what Dementia is; discuss the physiological and psychological changes that occur, and explore the person’s needs. Also I will discuss the role of the carer and multidisciplinary team in relation to caring for the person.
I will use the Internet, tutors notes and hand-outs for this project.
What is Dementia?

These specific physical changes associated with Dementia can be something as simple as lack of personal hygiene, wearing dirty clothes, forgetting to eat or drink which can lead to malnutrition and dehydration. They can also become incontinent and their balance might also be effected. Their mobility can be impaired as the Dementia progresses. Their ability to swallow and sleep pattern can also be effected. The carer in conjunction the MDT must have a unified and personal centred approach to the person with Dementia. They need to monitor the person to make sure their condition doesn’t change from the care plan. If there are any changes in the person, the care plan needs to be adjusted accordingly. The carer needs to know how to apply the method of care that is needed. The carer needs to have empathy, patience and understanding for the person with Dementia. The carer needs to get to know the likes and dislikes of the person. A person with Dementia can often present challenging, aggressive behaviour; there is always a reason for this. Perhaps they are in pain and can’t express or explain what is wrong with them. The carer needs to observe and report any changes. Care plan needs to be reviewed every 3
