Description Of A Discourse Community

The purpose of this essay is to connect a discourse community I am a part of and finding a genre that focus on reading and writing. Every human being is a part of discourse community. A discourse community for me is where I am working at. I have been part of this discourse community for three years now. I work at Valle Optical it is an eye care clinic. It is located in Southwest Detroit. I am an assistant. Being an assistant means having to assist patients with any questions they have in regards to eye care, documents, translating. Every patient has had to fill out a patient form to be helped in our offices. I am the person who gives these forms to new patients every day. The patient form is a genre of reading and writing, because in the community I am a part of I have to check their medical history and personal information to see if they can get help here at the eye clinic I work for. The patient form has the format that makes it easy for patients or any individual to distinguish right away what goes where. The format is basic it has bold words that stand out when a new set of information needs to be given. The form is organized from left to right, has the
It starts with the first, middle, last name of the patient. The form asks to provide the date of birth of the patient. Then it asks to write down the patient’s contact information like the phone number where the office can react the patient. In that section, the form also requires to state where the patient lives, so the address. The eye care clinic requires that information because sometimes after we take care of them as after the consult what ever medical the patient has them cover needs to know they went to the eye care and if the patient needs reminders of the next appointment we send them reminders or call them through the information they gave
