On a weekday morning, hundreds of people running out of City Hall Shopping lobby coughing, an estimated of at least 400 casualties are involved. City Hall MRT is an underground station located in the civic district in downtown core within the central area and serve as an underground passage connecting various shopping mall and office building together. Any disaster that occurs in this area will expect to have a huge impact and large amount of casualty.
Being a student in Advance Emergency Nursing Course (AENC), I am expected to be able to manage and plan during disaster, knowing the resources available, able to identify the sign and symptoms of chemical agent used and manage these casualties, providing them necessary medical treatment.
Disaster Nursing and Management
Definition of disaster refers to natural or man-made hazard resulting physical damage, destruction, loss of life or drastic changes to the environment. (Rainey, 2007) In such situation, the Singapore Civil Defence Force (SCDF) will be the first to be activated and subsequently various government agencies like Singapore Police Force (SPC), Singapore Armed Force (SAF) and others will be activated as well.
Chemical Agent
People have been using chemical agents for thousands of year from Ancient Greeks attacking enemy using toxic smoke to today as chemical weapons to be use in wars. In order to handle chemical agent attacks, government having been training how to handle and response appropriately during attacks. (Smith, 2013) Common chemical agents used in attacks are usually pulmonary irritants, nerve agent and vesicant agent. Respectively, they can cause difficulty breathing, brain damage and painful blister on skin.
Majority of the casualty are presenting with initial coughing symptoms as this indicates pulmonary irritant which can be caused by either nerve or chemical agent. Subsequently casualty further developed symptoms like diarrhoea, urination, miosis, bradycardia,
