Examples of models of practise that underpin equality, diversity and inclusion in my own area of responsibility are:
Person centred care planning – this model concentrate on the person as a unique individual. It places the person in the centre of their needs-physical, psychological, social. It also offers the person full involvement in their care. It is vital that all care and support is personalized and that individuals are supported to have control of own lives.
