SCHOLARSHIP APPLICATION FORM FOR UNITEN 2015 FOUNDATION PROGRAMME
Applicants are required to read and understand the information below.

Gained admission to the 2015 Foundation Programme
Excellent academic results in SPM with As in at least 7 subjects
Not bonded to or in receipt of any financial aid or scholarships from other educational institutions or organizations
Added advantage for applicants who are active in sports and/or co-curricular activities
VALUE OF AWARD
•
Each scholarship award covers tuition fees, allowance and accommodation fees. The scholarship will be for the minimum period required to complete the Foundation Programme and thereafter,
Degree Programme and shall not exceed RM110,000 in total for the entire duration of study.
SURETIES AND SERVICE BOND
•
Successful applicants are required to provide 2 sureties prior to the execution of the scholarship agreement and serve the Group or any of its related companies for a minimum of 5 years upon graduation. TERMS & CONDITIONS

Subject to the terms and conditions stated in the Scholarship Agreement.

All application forms must be submitted to UNITEN’s Sponsorship,
Career and Alumni Centre.
2.
The closing date is on 30 May 2015. Applications received after the closing date will not be entertained.
3.
Please attach a copy of your Admission Letter from UNITEN together with your application form.
4.
Applicants are NOT required to enclose any supporting documents
(e.g exam results, school testimonials or parents’ income statements).
5.
Applicants are required to read and understand the Notice Pursuant to the Personal Data Protection Act 2010 on Page 7. Your consent is required in order for us to process your application.

Only shortlisted candidates will be called for interview.
