1) There are several methods an employee can use to file a complaint with the EEOC including in person, telephone, or mail.
If you choose to file in person you should bring all evidence to your appointment. Once you arrive at your appointment be prepared to take an assessment to assist in evaluating if the EEOC is right for your type of complaint and if the EEOC is the right department to file your complaint.
Once you have decided that you want to file a complaint with the EEOC, you must fill out a four-page intake form that includes your name, sex, race, date of birth, contact information, phone and alternate phone number, country of birth. Along with the name, contact information and the number of people you believe have discriminated against
