T/R Blade Assy., P/N: 355A12-0031-01 to -14 & 355A12-0040-00 to -08
Check Tail Rotor Blades for Condition.Comply with DGAC AD F-1984-045-022 R4; SB 05-06 R6, Para. 1.C.1, Check Tail Rotor Blades (without
Removal) for abnormal noise, not a metallic sound but sound of broken fibres as follows: Simultaneously hold both blades at 50cm distance from hub centre line & bending them simultaneously inward & then outward & check for abnormal noise.
Comply with EASA AD 2007-0138; ECF SB 05-00-38 R2, Para. 2.B, Inspection of Tail Rotor Blade Skins in Tab Areas for cracks.
Check Laminated Half Bearings for delamination, deep cracks & protrusions.
Check See-Saw Hinge for play at Swivel Bearings, condition of Rubber on Bearing Outer Cone & check no cracking or protrusions exist.
Check TGB Cowling for condition & security.
Check Pitch Links & Pitch Change Mechanism for condition, abnormal wear, play & security.
TRH Pitch Change Spider, P/N: 350A33-2004-00 to 06, Pre. & Post Mod.

Comply with requirements of AS355 SL 1169-63-93 , Inspection of
Universal Joint Safety Pins for security, integrity & correct installation.
Check Hydraulic Reservoirs for security, correct levels & filler caps secure.

Inspect Hydraulic Pump Magnetic Plugs.
Check Hydraulic Flexible Pipes for condition, security & leaks.
Check Hydraulic Servos for condition, security & leaks.
Check Flight Controls (where visible) for condition, security & damage.
MGB Fwd. & Aft. Suspension Bars, P/N: 355A38 -0040-02/-03/-04/-05 Check MGB Mount Suspension Struts / Bars for condition, security
& End Fittings Checks. Checking the Ends for longitudinal displacement of the Spherical Bearings. Also check that the PRC in not cracked or protrudes beyond the end of the Bars.
MGB Suspension Bi-Directional Cross Beams, P/N: 350A38-1018-All, Pre. Mod. 072720
Visually check both the upper faces of the cross beam for evidence of cracks.
Check MGB Suspension Cross Beam for security. Check Elastomeric Mounts for condition & contamination.
Check Transmission Decking for cleanliness & leaks.
Check Engine Oil Tanks for security, correct levels. Check Mag.

    According the NTSB, damage was noted to the thrust reverser, left wing, and fuselage of the aircraft (2006). Further examination revealed the number one engine intermediate pressure turbine case rupture had left a gap between the intermediate and low pressure turbine cases and fractured the disk rear drive arm and all blades were missing from the blade slots. During this initial inspection it was also noticed that the high pressure and intermediate pressure turbine bearing chamber external vent tube had two holes burned through outboard of the intermediate pressure turbine connection. According to Anderson “Most turbine failures are the result of over-temperature which is usually caused by hot starts, compressor stalls, or excessive fuel” (2009, p. 8.60).
