Eulogical Reflection
On 8/9/6 I met Ms. Wharram at the office of Dr. Sorscher. Ms. Wharram returned to work on 7/11/16. She continued to attend physical therapy and wears her brace while at work. The doctor’s office was full and our wait was long. During our wait Ms. Wharram was able to outline all of her current problems. She reports the knee has a small round “pouch”, she is unable to kneel on the right knee, and there are 2 spots to the inner and top of the ankle that she can feel the hardware. She is concerned that this will bother her when wearing winter boots. There is also pain in the ankle when she is trying to ambulate using a heal toe walk. Her ankle swells, and she has a depression on her shin that is numb and looks “funny”. She reports she …show more content…
She expressed concern about walking with a limp and what would happen when she gets older. Overall she says she is very upset about her progress or lack thereof. X-rays taken today showed good bone healing, good alignment of the hardware and no areas of concern per Dr. Sorscher. Dr. Sorscher took time to examine all of the areas Ms. Wharram was concerned with. He said the knee will be painful to kneel on for a while yet, the “pouch” on her knee is also on the left knee, and the swelling will get better. He said he cannot tell what will occur in the future. He wants her to continue to work with physical therapy and do her home exercise program. He refilled the Norco but, told her that she should try some extra strength Tylenol and see if that will help. Wharram will return to fulltime work on 8/23/16.

ASSESSMENT

Ms. Wharram still is resistive to returning to full duty work. She has increased her therapy from 2 times a week to 3 times and hopes this will help with pain and range of motion.

PLAN/RECOMMENDATIONS

1. Contact Ms. Wharram every 2 to 4 weeks for an update on her medical status.
2. Attend follow up appointment with Dr. Sorscher Orthopedic surgeon on 9/20/16 for a treatment plan, work status and projection for MMI.
3. Update adjuster after appointment, Ms. Castle will update the employer.
4. Facilitate physician

    She is doing a little bit better. She has been taking some anxiety medication and sleep medication that has been helpful. She is also taking Norco. She alternates gabapentin with Lyrica and takes tramadol with good relief and tolerates it well. Lyrica provides better relief, but it was mentioned that she will take gabapentin since she was difficulty getting her prescriptions. She has aching pain from the elbows down to the wrist with numbness and tingling in the 4th and 5th ginger and hypersensitivity of the entire arm as well as weakness. Pain levels are 7-8/10 without medication, coming down to 2-3/10 with medication. Her pain is worse with walking, bending, and lifting. It is decreased with sitting, standing, lying down, and medication. Functional improvements, she has been more active this past month. She has been exercising more with her medication.     The 2mWT was administered at session 4 and 7 revealed improvement in the patient’s endurance by ambulating 96 feet in 2 minutes at discharge compared to 78 feet at re-evaluation (Table 3). The improvement in the patent’s distance (18 feet) did not met the published minimal detectable change (MDC) value for individuals with MS (MDC=19.21 meters/63.02 feet) (Gijbels). An improvement in the patient’s fall risk status was indicated using the BBS. The patient improved from high risk at initial evaluation to medium fall risk at discharge exceeding the reported MDC for an initial score of 0-24 points for the elderly (MDC=4.6 points) (Donoghue). The patient scored higher, improving in all items except for standing with one foot in front and standing on one leg where no change was made. There was no significant improvement on the patients MS walking scale score; her high score indicates that she has a strong perception that MS greatly impacts her ability to walk. No MDC values have been reported for the MSWS-12. Lastly, the patient’s score on the LEFS improved but remained under the published MDC value for various lower extremity injuries (MDC= 9 points) (Binkley). The patient reported on the LEFS at discharge less difficulty standing, preforming light activities around the house and walking between rooms. All STG and LTG were meet at the time of discharge. The patient’s ability to complete transfers in and out of bed and chairs, stand for prolonged periods of time, static and dynamic standing posture and ambulate greater distances significantly improved with less fatigability and greater ease. Her patellofemoral pain was unable to be completely managed be but reductions in pain were achieved along with improved alignment of the left patella thus minimizing lateral deviation of the patella.
