This Kaiser Permanente research training is presented by June L. Rondinelli, RN, MSN. It is the twentieth video in a series of twenty that goes over nursing research. In this online lecture, she focuses defining outcomes research describe the components of Donabedian’s Model of Quality Health Care, listing nurse sensitive outcomes, and discussing places to find practice guidelines. Overall, the presenter discusses outcomes research as a way to evaluate change in nursing practice. Watching this presentation can help to strengthen the understanding of what research is and the purpose of why it is conducted.
As outlined by Rondinelli, outcomes research was established to look at the end point of patient care. It is a tool that evaluates the results of something that is researched. For example, it can be looking at the status of people after receiving care and the methods used to attain wanted results. Outcomes research is the use of evaluation methods, along with epidemiology and economic theory. In addition, outcomes research is helpful in impacting the creation of policies and procedures, guidelines, and even public health policies.
Rondinelli shared about the Donabedian’s Model of Quality Health Care. The model discusses three major connected …show more content…
concepts: structure, process, and outcome. More explicitly, the Donabedian’s Model of Quality Health Care can be seen as an equation where structure in addition to the process yields outcomes. This model was established in 1972, and during this time healthcare providers and researchers assessed each part of this equation along with the links between the concepts behind “structure,” “process,” and “outcomes.” Rondinelli describes structures as organizational characteristics. For instance, a structure can be health care personnel, healthcare facilities, and healthcare equipment and resources. Processes are identified as diagnosis, prevention, interventions, and patient education. Examples of patient outcomes includes clinical end points like signs and symptoms, lab values, and mortality rates. It could be the functional status, general well-being, and satisfaction with care. Interestingly, it can also come from looking at nurse satisfaction.
There are many nurse sensitive outcomes that have resulted from evaluating outcomes research.
Most of these outcomes are described as evidence-based practice models. Common steps for formulating these models is by performing analytical reviews of the research literature. It can be from obtaining baseline information from outcomes research. Implementation of the information and evaluation of the results occurs in evidence-based practice models. It is also significant to incorporate findings into the clinical setting. The different evidence base practice models include the Iowa Model of EBP to Promote Quality Care, the ACE Star Model, and Stetler
Model.
As described by Rondinelli, places to find practice guidelines. These guidelines are systematically developed statements about a plan of care for a specific set of clinical circumstances involving a particular population. Examples can be found on the Center for Disease Control (CDC) and National Guidelines Clearinhouse website. Sources also include professional organizations such as the American Heart Association and the American Diabetes Association. Lastly, specialty professional organizations could be a good source for practice guidelines.
Finally, outcomes research and evaluating change in nursing practice is crucial to the field of nursing. Consideration of the outcomes contributes to nursing in that it helps to provider safer and better quality care overall. Lastly, understanding of outcomes helps to grow the field of nursing and helps to grow the research being done in nursing today and in the future.
