To be able to make accurate decisions, the objective needs to be clear otherwise the outcome may not be as required. The scope is similar in the fact that it should outline what the decision is to be made in regards to. It could be a statement in regards to a number of objectives. The success criteria could be a number of outcomes that need to be achieved. If it was from a system, check how accurate the system is. How accurate is the gathering of the information? It would be ideal to do some form of risk assessment in regards to the information. If it was a person, can you get them to sign a statement stating the information is correct as to their knowledge? It may be that you have to investigate the source of information and assess how accurate it is.

This is dependant on the decision that needs to be made. You need to gather as much evidence as you can in regards to why you think something will hamper targets/quality standards. This way you will have evidence to back up anything that you address. Ideally, as a manager if you have anything that hampers the above, you need to investigate what is needed to stop any decision from hampering the achievement of targets and quality standards.
