The patient is a 54-year-old female who continues on Paxil CR currently at 37.5 mg p.o. daily. She started this medication in June of 2013. Unfortunately, since being on this medication. She has gained significant weight and continues to gain weight. She states she really is not eating any differently, is exercising, if anything, more. She does tell me as well, however, that she knows that part of this is being perimenopausal. She also tells me that the medication helps tremendously with her anxiety. In the past, we have talked about her weight gain and the fact that she is so pleased with the benefit to her anxiety, that she wants to continue it. In January, she did try going on Wellbutrin XL 150 mg per day, but at the same time decreased her dose of Paxil to 25 mg. With these two changes done at once, she noted increased anxiety after one week of being on Wellbutrin and stopped the Wellbutrin and went back to the 37.5 dosing of the Paxil CR. The patient has been on multiple other medications including Lexapro, Zoloft, BuSpar, with side effects on all of these. She has two under her bra on the left side that have become polyploid. They are irritated. They do not bleed. She also has several on her back.
The patient denies any personal or family history of seizure disorder.
Objective
Vital Signs
Blood pressure 120/78. Pulse 90. Weight 174 pounds, which is up from 165 pounds at her visit in February of 2015 and up from 136 pounds in June of 2013.
General
Patient is alert, oriented, no acute distress, good eye contact, speaking clearly and logically.
Skin
With multiple red, slightly raised papules on her anterior chest, two under her bra have formed polyploid formation with narrow stalk. She does have several benign lesions on her back, but two of which are skin
