1. Physical Hazards :

1.1 Noise – some machinery was found to be in the hearing protection advisory range yet no signage was found, potential hearing damage.

1.2 Wet floors – constant cleaning of machinery was leaving pooled water on the floor causing slip hazards

1.3 Trailing cables – to jet wash and FLT batteries causing trip hazards

1.4 Office chair broken and unsafe – caster missing from one foot and broken arm rest, falling, nip/pinch point hazard

1.5 Flickering monitor on IT system – potential damage to eye sight

2. Transport Hazards :

2.1 FLT movement – High volume of FLT movement, internal and external as well as pedestrian foot fall, potential for collision

2.2 HGV movement – Vehicles were found to be not following traffic management plan.

2.3 Skip loading / unloading was taking place but external contractors could provide no risk assessment or safe sytem of work,

2.4 Roadways in need of repair in some areas (pot holes)

2.5 Good practice – all flt drivers were appropriately trained with 3 year refresher training all in date

3. Machinery Hazards:

3.1 Compactor not locked off during maintenance. – serious hazard, heavy machinery, potentially fatal.

3.2 Baler hopper weld cracked – Weld could fail and cause injury to any near by persons.

3.3 High pressure hydraulic hoses not guarded – Leak dampers are standard site procedure to prevent such issues as high pressure injections.

3.4 PUWER assessment had not been reviewed following additional padlock safety measure recently added.

3.5 Emergency stops identification signs on compactor 1 were found to be dirty and not immediately obvious
