The patient stated he had gone to the pharmacy when he got home to get it filled immediately. He dropped the prescription at the pharmacy expecting to be able to collect it the next day. However, when he arrived to collect his prescription, the pharmacist told him that, while what had been completed on the prescription was correct, the issuer of the prescription had failed to include the dose for the medication.
