Introduction to record management Happy Health Medical Clinic prides itself in being keeping up compliance and organization. Records will soon be making the switch to electronic formatting.
Until the switch we are dedicated in teaching others the proper handling of all medical information.

We are a new practice that has six doctors on staf
All files sent to x-ray must be retrieved.
These are the original records and not copies.
Record Location
It is essential to know where a record is located so personnel can access or add information to a record.
It is important to get all new or updated information into a patient chart in a timely manner to prevent loss or payment complications. Duplication of the same medical record should never be done to avoid confusion and to protect patient confidentiality.
Steps in locating records Basic patient information can be found in the electronic data base the patient information is located in the chart.
All patient files are alphabetically filed.
The office has centralized as well as decentralized records. All records should never be left on a desk unattended but filed immediately after information is inputted.
Creating New Records
Records are filed alphabetically by patients last name first then patients first name.
Basic information should be inputted into the computer
Sometimes reoccurring last names can be confusing. It is important to check the patient’s DOB to keep patient information accurate. Example Johnson Ashley and Johnson Ashlee
Loose forms and care reports Clinical data that the doctor needs to see goes into the doctor’s mailbox. After it examined by the doctor it goes into the temporary holding area until filed into the patient chart.
