Chapter 20 – Heart and Neck Vessels
Direction of blood through the heart-
VENA CAVA
RIGHT ATRIUM
RIGHT VENTRICAL
Pulmonary arteries lungs pulmonary vein left atrium
Left ventrical aorta Areas where heart valve closures can be heard-s1, loudest at APEX, S2 LOUDEST AT BASE OF HEART.
Assessment of carotid arteries in an older patient-USE CAUSION, LIGHT PALPATIOIN, ONE SIDE AT A TIME. ASCULTATE EACH CAROTID ATERY FOR PRESENCE OF BRUIT(SWOOSHING SOUND)
INSPECT JUGULAR VENOUS PULSE
Stethoscope how to listen to heart sounds- use bell (LOW PITCH) DIAPRAGM 1ST !!( HIGH PITCH)
Diaghram in a Z PATTERN BASE OF HEART,ACROSS, DOWN, APEX. TUNE OUT DISTRACTIONS, CLOSE EYES , CONCENTRATE,,LISTEN TO 1 SOUND AT A TIME.
Area of palpation of apical impulse-midclavicular line, 5th intercostal space, almost under left nipple.
Estimate the Jugular venous pressure-USE ANGEL OF LOUIS(STERNAL ANGLE) AS A REFERENCE POINT, AND COMPARE WITH HIGHEST LEVEL OF VENOUS PULSATOIN. HOLD A VERTICAL RULER ON THE STERNAL ANGLE. ALIGN A STRAIGHT EDGGE TO THE HIGHEST LEVEL OF PULSATION.
And under 2cm HIGHEST LEVEL OF PULSATION, ATTATCHED TO RIGHT ATRIUM.
Thrill-rushing PALPABLE vibration indicative of a heart murmer.
Precordium- area over the anterior chest overlying heart and its great vessels. PROTECTS HEART.
Chapter 21 – Peripheral Vascular System and Lymphatics
Inguinal lymph nodes assessment for infections-palpate in groin, infection present
If SWELLING occurs in nodes OR ENLARGEMENT
Lymphatic system of a healthy 3 year old child-large and palpable even when ill
The presence of palpable lymph nodes
Lymph nodes are relatively large in children, and the superficial ones often are palpable even when the child is healthy.
Capillary refill time-2seconds
Positive Homan’s sign and thrombophlebitis (possible complications)-postitve homans sign calf pain when performing knee flexion or
