Ms. Marsha Geddings contacted the Client Assistance Program pertaining to her meeting with her new counselor on Thursday, September 15, 2016, at 11:00 a.m. I reviewed Ms. Geddings’ case and I would like to obtain some additional information from the Lexington Area Office. The Client stated she has some anxiety about her meeting to develop her IPE. I am getting the impression from the case notes that the Area Office Supervisor has and is continuing to address the Client’s concern by reassigning the case to a different counselor. Lynn had a unrestful night and into the morning of 7/15/15. The morning medication was adminstered at 7:00 pm. Although Lynn had been given her morning medication and had eaten breakfast, she began requesting and demanding her 1:00 pm medication. At approximately 8:00 am, Lynn’s behavior escalated and she began to attack the support staff, DSP Clark and DSP Seay. Lynn pinched, scratched, chased, and threatened harm to both DSP’s. Lynn would demand medication and then have moments of sobbing cries, in which she would beg for an overnight stay at a hospital. A phone call was placed to Mobile Crisis at approximately 8:30 am. Transitioning to adulthood can be stressful and challenging for all, but for those diagnosed with Autism Spectrum Disorder (ASD) and their caregivers transitioning to adulthood can provoke feeling of uncertainty or even fear. People with ASD function at different levels and require varying degrees of care. There should be an individualized educational plan (IEP) established by age 16 containing postsecondary goals related to training, education, employment, and independent living skills along with the transition services needed to assist in reaching those goals referred to as individualized transition plan (ITP) (Heward, 2013). Defining the needs of young adults with ASD should start with listening to the individual and helping him or her plan for the life he or she wants. When establishing an ITP, goals in education/training, employment and independent living; type of support needed on a job; residential alternatives; and interventions for any behavioral problems should be addressed.…

I am requesting 29 days of leave from 23 June-21 July 17, which is outside of the authorized 14 days of leave. This request is due to the following reasons: I arrived to brigade from 96th TC, 3 weeks prior to the departure date for this deployment. Due to time constraints, I wasn’t able to take block leave because I had to complete SRP, store my household goods, and obtain my RFI. I informed my section OIC and NCOIC of my request to take leave pass the 14 authorized days. The worker complained of mismanagement of her return-to-work programme, including inappropriate exposure to the client that had assaulted her, physically unsuitable duties, and poor communication between staff who appeared unaware of her problems and needs. The worker reported constant and moderately severe central lower back pain that radiated up her spine to the dorsal back and down through the groin and core of her thigh on the right only. The report notes no lower limb neurological symptoms, but right knee pain, swelling and clicking and similar symptoms in the left that the worker attributed to overcompensating. The worker reported being significantly physically debilitated because of her aches and pains, and had a reduced tolerance to activities including sitting, standing, walking, and exerting force with her upper limbs. The assessment states that the worker experienced significant limitations in her capacity to undertake domestic chores, and fatigue after work or those limited domestic chores was recurrently reported.
