How To Write An Example Of Narrative Report
STATE WHAT YOU OBSERVED

On 04/17/2015, at approximately 0119 hours, I was on routine patrol in a marked police vehicle. I observed a red in color sedan travelling North on State Route 924 (Main Street). While the vehicle was in motion the operator failed to activate the headlights, driving without headlights during dusk. I proceeded to turn around, to travel in the same direction as the described vehicle. I activated the emergency lights stopping the vehicle at Main and Washington Streets.

I approached the driver and requested his license, registration and proof of insurance. Before the operator supplied the documents I requested he asked why he was being stopped. I informed him that it was because he was driving without his head lights on. While speaking with the male I noticed red glassy blood shot eyes, a slow slurred speech and an odor of an alcoholic beverage emitting from inside the vehicle. I question the operator how much he has been drinking tonight. He related he wasn't and just got off of work. I asked again and he stated "maybe two beers". The operator then supplied the documents I previously requested. At this time the operator was identified through his Pennsylvania driver's license as Richard W. Eltringham (from here on known as the defendant).

I transported the defendant to the hospital where a blood sample was drawn from his right arm. The defendant's arm was wiped clean with a sterile non-alcoholic prep pad prior to venipuncture.

The toxicology report indicated positive findings of: 0.181% (Percentage WT/VOL

A Missouri police officer stopped Tyler McNeely after observing it exceeding the posted speed limit and repeatedly crossing the center line. The officer noticed McNeely’s bloodshot eyes, his slurred speech, and a smell of alcohol on his breath. McNeely performed poorly on a battery of field sobriety tests, and he declined to take a Breathalyzer test. When McNeely indicated he refuse a breath sample for testing, the officer took him to a nearby hospital for blood alcohol test. The officer explained to McNeely that under Missouri’s implied consent law, refusal to submit voluntarily to the blood test would lead to an immediate one-year suspension of his driver’s license and could be used against him in any future prosecution. The testing of the blood indicated that the blood alcohol level was significantly above the legal limit. McNeely had challenged the blood test evidence claiming that there should have been a search warrant before ordering a blood sample.…

Armando Schmerber, the petitioner, had been arrested for drunk driving while receiving treatment for injuries in a hospital. During his treatment, a police officer smelled liquor on petitioner's breath and noticed other symptoms of drunkenness so the officer ordered a doctor to take a blood sample which indicated that Schmerber had been drunk while driving. The blood test was introduced as evidence in court and Schmerber was convicted. While in the hospital the petitioner was informed that he was entitled to counsel, that he could remain silent, and that anything he said would be used against him. At the officer's direction, the physician took a blood sample from petitioner despite his refusal on advice of counsel to consent thereto. A report of the chemical analysis of the blood, which indicated intoxication, was admitted in evidence over objection at petitioner's trial for driving while intoxicated. Schmerber was convicted, then filed an appeal against The State of Califoria.…

Ptl. Rivera and I conducted a search of the vehicle and located a nearly empty bottle of UV vodka in the rear compartment of the vehicle. All three subjects submitted to a portable breath test and all three results yielded a BAC of .00. Martin stated that he shares the vehicle with his older cousin who is of age. The vodka was poured out and disposed of. Ptl. Rivera further located a Gillespie Lieutenant badge in the glove compartment of the vehicle. Martin stated that his grandfather is a retired police officer from Gillespie. I advised Martin that the badge would be seized and that his grandfather would have to come to the Bethalto Police Department and speak with me to have the badge returned to him. I continued to scold the three for having alcohol in the vehicle and to be trespassing on a road where there is a visible “No Trespassing” sign in the road. I told Martin to take Marrujo and Metcalf home immediately. I told Marrujo and Metcalf to tell their…
