RE: Patient Transfer—Ms. Angela Tennyson
Dear Mr. Henry Madsen,
Please consider this letter as advanced notice for receiving a patient transfer. Ms. Angela Tennyson will be transferred into your custody on October 20th, 2013. I am a Registered Nurse at London Health Sciences Centre (LHSC), who has been providing care for Ms. Tennyson for the past month. I would like to see her have a smooth transition to your facility. The intent of this letter is to familiarize your staff with essential information regarding Ms. Tennyson’s needs and healthcare plan, while taking into consideration her physical, mental and emotional well-being. I trust that this information will not only help you, but also enhance the quality of life for Ms. Tennyson.
Since her physical assessment in 2010, Ms. Tennyson was diagnosed with rheumatoid arthritis, primarily in her knees. Her mobility has become restricted, which makes her susceptible to lethargy. In order to increase elasticity and stamina, I advise physical therapy. Our current Physiotherapist has made significant progress, and thus Ms. Tennyson should continue to make improvements to her mobility. Ms. Tennyson was diagnosed with Huntington's disease in 2007. The outcome of this illness causes mild dementia, and disorientation. It would be therapeutic for her to reflect and reminisce about her childhood through photos and memorabilia, as well engage in her favourite hobbies. Ms. Tennyson’s spatial awareness and cognition is restricted, which causes her to be irritable at times. Please be wary that she will easily become agitated and this may aggravate her previously diagnosed hypertension.
Ms. Tennyson suffers from depression and general anxiety; therefore, monitoring her behavior for indicators of these symptoms is a concern. These conditions had lead us to provide psychological counseling
