Background:
According to CDC, based on 2008 to 2012 data, Human Papilloma Virus (HPV) causes about 30,700 cancers per year. It causes more than 90% of cervical and anal cancers, around 70% of vaginal and vulvar cancer, more than 60% of penile cancer and more than 70% of oropharyngeal cancer. Among them, majority (>70%) has been linked to HPV 16/18, which can be prevented by bivalent, quadrivalent or 9valent vaccine. Healthy People 2020 objectives is to increase HPV vaccine series completion for females aged 13 to 15 years to 80% by the year 20201. Random public and private school will be selected of Chicago area. Permission to visit the school will be obtained from board of respective school. Researcher will visit the classroom of students with 13-17 years of age and explain the process; they will take a sealed envelope of consent form and validated questionnaire to their homes and provide it to parents or guardian. First section of questionnaire contains sociodemographic information and other three sections will address the knowledge and compliance of HPV vaccine4. Researcher will return to school the next day to collect the filled questionnaire and if its <80% then she will return to collect the remaining ones on subsequent days after providing additional invitation to participate. Parents of respective students with contraindication to vaccine or who did not sign the consent form or did not complete the questionnaire will be excluded from the
