To introduced new viewpoint into work situation, in effect, to implement change. Change refers to process that brings about alteration in behavior or substitutes one way of behavior for another. Nursing in itself can be seen to be an agent of change. It hopes to bring about change in patient or client behavior, such as changing dependence into independence, fear into security and so forth (Pearson,1996). The four steps used in problem solving or the nursing process including planning, implementing and evaluating nursing care, and also describing with explaining response to outcome
of care. Each different model and nursing theory guides each of nursing process, it guide nurses to collect, organize, and classify patient data and to understand, analyze and interpret patient health situation. Firstly, assessment the situation to collect information and problem to formulate nursing diagnose then identified the problem, plan a goal to be achieve and action to settle the problem by arranged group discussion and teaching to get greater understanding of the model, secondly to do appropriate types of nursing intervention, regularly monitor the change, judge its effectiveness by previously agreed methods. Lastly, patient outcomes to be included in the care plan.
