CM spoke to Tamicka Goode (caregiver) in regards to scheduling an initial meeting with the family. Ms. Goode provided CM with available dates and time for initial meeting; CM and Ms. Goode scheduled initial meeting for Thursday, 3/23/17 at 5:00pm at the family’s home. CM informed Ms. Goode that CM coworker will be in attendance. CM also provided Ms. Goode with a brief description of the FSO program. CM asked if Ms. Goode would be interested in having the FSO come to the initial meeting. Ms. Goode accepted FSO services and CM explained to the caregiver that FSO will attend the initial meeting as well. CM informed Ms. Goode that at the initial meeting paperwork will be signed and both she and youth need to be present. CM confirmed

    At this time, a full overview of this case was given. Which is as follows: I explained the client is in Star plus waiver, a 30 day letter was issued from ASH on 1/6/17 and an MDU slot was requested on 1/6/17 and then again on 1/12/17 as I did not get a returned email that Cindy Pollan had already retired. Nor did I get a returned phone call from her voicemail, which was still active. After speaking with other DADS/HHSC contacts, I got your information and we spoke on 1/25/17. This caused an issue, the client cannot return home, ASH needs to dischrge, yet he cannot enroll until 3/1/17 because of the Star plus waiver. I called seeking guidance on if the waiting until the 1st of the month was an HHSC rule that could be reviewed and changed for this special case or was the rule from Medicaid.…

    At this point in the conversation Pamela raised her voice and began to discuss issues that she felt she needed assistance with.
