As for NS Process, NCLEX Testplan & Blooms Taxonomy: for each test question, there should be an ‘X’ placed in one of the columns under each of these categories (see descriptions below). Ex. If the question is having the student recall a piece of data collection about a psychiatric patient directly from their notes, an ‘X’ would be placed in the following categories Assessment, Psychosocial Integrity & Knowledge.
DISCRIMINATION: (For Faculty Use Only) the degree to which an item discriminates between students with low achievement and those with high achievement. POSITIVE DISCRIMINATION: more often, the item was answered correctly by those with high achievement on the test.
NEGATIVE DISCRIMINATION: more often, the item was answered correctly by those with low achievement on the test.
DIFFICULTY: (For Faculty Use Only) percentage of students who answered the item correctly. <30% = difficult question.
Instructor should make sure the content is supplied to the students, possible remediation 30%-70%=ideal question
>80%=easier question. Instructor should look at this question for review to possibly make a higher level question.
ASSESSMENT: Obtaining, validating, and communicating subjective and objective client assessment data including patient history, consultations, lab findings, pharmacological requisites, and the nurse’s physical examination. Also involves the nurse’s determination of the congruency between the client’s needs and the ability of health care team members to meet such needs.
ANALYZING: Selecting relevant data and drawing inferences and conclusions to identify actual and/or potential problems that require nursing assistance. Includes identifying nursing diagnoses and communicating results of the analysis.
PLANNING: Making plans with the client to set goals and establish outcomes to deal with the identified problems. Includes prioritizing, communicating a plan, and collaborating with other health care team members to deliver comprehensive care. IMPLEMENTING: Includes nursing behaviors and actions such as communicating, teaching, performing, or assisting with activities of daily living, and informing client and/or family about health status in order to achieve the established goals and expected outcomes.
EVALUATING: Gauging and measuring the client’s response to nursing actions and the movement toward or away from identified goals. Includes comparisons of actual outcomes of therapy with expected outcomes, evaluating compliance with prescribed or proscribed therapy, describing response to therapy or care, and determining the extent to which client outcomes have been achieved. Also involves the modification of plan, as indicated, and the re-establishment of prioritized care needs.
SAFE EFFECTIVE CARE ENVIRONMENT: The nurse promotes achievement of client outcomes by providing and directing nursing care that enhances the care delivery setting in order to protect clients and other health care personnel.
HEALTH PROMOTION AND MAINTENANCE: The nurse provides and directs nursing care of the client that incorporates the knowledge of expected growth and development principles, prevention and/or early detection of health problems, and strategies to achieve optimal
health.
PSYCHOSOCIAL INTEGRITY: The nurse provides and directs nursing care that promotes and supports the emotional, mental and social well-being of the client experiencing stressful events as well as clients with acute or chronic mental illness.
PHYSIOLOGICAL INTEGRITY: The nurse promotes physical health and wellness by providing care and comfort, reducing client risk potential and managing health alterations.
KNOWLEDGE: Exhibits memory of previously learned material by recalling fundamental facts, terms, basic concepts and answers about the selection. Commonly used words include: what, why, choose, define, match, select
