We are writing this letter to present the facts of this claim on behalf of our client, Johnnie Mae Williams, in an attempt to settle this claim amicably without the necessity of litigation. The information we supply herewith is a brief summary of the facts that would be presented at the trial of this matter.

To assist you in your evaluation of this claim, enclosed please find the medical records and billing statements of our client, Johnnie Mae Williams, who sustained permanent injuries after slipping and falling on March 15, 2016, as a result of the negligence of your insured, Reef Life Laundry. Williams presented to Advanced Diagnostic Group to undergo an MRI of her cervical spine. The results revealed:
1. There is mild spondylosis with anterior osteophytes at C4-5, C5-6, C6-7. At C5-6 there is mild disc space narrowing.
2. At C3-4 there is central disc herniation superimposed on disc bulging. The herniation protrudes 2.5mm.
3. At C4-5 there is posterior central disc herniation with mild inferior extrusion. The herniation is superimposed on disc bulging. There is edema present within the herniation which is consistent with recent acute or subacute injury. AP diameter of the canal is 8.5mm and there is mild to moderate central canal stenosis. There is right foraminal stenosis with small unconvertebral spurs.
4. At C5-6, there is disc bulging impressing the anterior margin of the thecal sac. There are unconvertebral spurs and facet arthropathy with mild bilateral foraminal stenosis.
5. At C6-7, there is central/right central disc herniation with mild inferior extrusion. The herniation protrudes 3.6mm and impresses the anterior margin of the thecal sac. AP diameter of the canal is 8.5 mm and there is mild to moderate central canal stenosis. The herniation is superimposed on disc bulging. There is facet arthropathy with mild bilateral foraminal

Williams treated as recommended, but continued to report the same symptoms. After review of her MRI, Dr. Michel from EMB further diagnosed Ms. Williams with Thoracic pain, Displacement of cervical intervertebral disc, Cervical Raficulopathy, and hand pain. As her condition worsened Ms. Williams began to developed numbness and tingling in her wrist, hand, and fingers. On July 26, 2016, Ms. Williams presented to UF Health Neurology for an electrodiagnostic evaluation. The study was abnormal. Diagnostic testing revealed that she was suffering with focal neuropathy affecting both median nerves at the level of the wrist. The results are

She was involved in a car accident 8 years prior to this, with no history of neck pain, and has developed progressive neck pain since the end of the last year.
