LMSW/SW Case Study

LMSW/SW Intern met with patient to conduct social work counsel. Patient reported that she was in domestic violence situation two months ago. Patient's stated that she was trying to move to Washington State during the summer. Patient's car got stolen and fount it later but damage. Patient is unable to proceed with the plan and looking for community resources. Patient plans to move to Alaska where her family are locted next summer. Patient is currently homless and living in her car, and looking for a shelter. Patient reported that she have a heart problem, which makes her disabled. LMSW provides patient with community resources
