Reason for Visit: Contusion of Left Ring Finger
S: TM closed a car door on his left ring finger. Reports pulsating pain 4-5/10. Denies any tingling, numbness, or loss of movement in his left hand or fingers. He is able to move all his fingers, and he is fine, and he has high pain tolerances per TM.
O: Left Ring Finger – edema, hematoma under the nailbed. Full ROM
A: Left Ring Finger Contusion
P:
Paint the nail with 10% povidone iodine (Betadine) solution. Using a 18 gauge needle, performed a trephination at the base of the nail.
Applied an antibacterial ointment and cover with a Band-Aid.
Inform the patient he will eventually lose his fingernail, until a new nail grows out after two to six months.
Disposition: Full Duty: Return to Clinic
