Mandatory systems are known to reduce the frequency of cases with errors besides of working upon a State Law. Innumerable hospitals agree with the mandatory, nonconfidential system that is governed by the State. This type of system is recognizable by processing internal incidents and reporting to a competency. In addition, nonconfidential system is acknowledged for encouraging lawsuits. In counterpart, the mandatory, confidential system, the error committed by employees, such as the physician, nurses, etc at a health facility are kept in secrecy. However, in case of serious injuries the hospital must communicate the State. Furthermore, the State is responsible for analyzing the cause of error and acting on the culpable. Even though the injury did not result on the patient’s death, but there was a branch for the error to be committed. Moreover, reporting mistakes is very important because it sets up a process on which data is collected and therefore reviewed and so forth on these risks can be discussed and evaluated to avoid the same error and to protect future patients. The Joint Commission agrees that once the error has been identified, dealing with a similar underlying problem the risks of committing the same mistake is reduced. On the other hands, patients want to be informed of the situation and they want to know that improvements were done with regard to the future patients. According to the nursing site, patients and the mandatory reporting expects that health facilities and organizations to acknowledge errors done by the staff, such as physicians, nurses, manager, and peers, and immediately they will report errors.
