Kholud Rajab Yousef is 29 years pregnant woman in her 32 gestational weeks, G: 8. P:4 admitted to antenatal wards at 6/12/2011 with complain of dizziness , shortness of breath, palpitation and fatigue, she also have history of varicosities since 5 years ago, which cause acute pain in her legs.

The pt. is on regular diet, not smoker, educational level is high school, she is house wife, her husband educational level is secondary school, he work as taxi driver , he is smoker, their socioeconomic states is poor, she rarely eat meats .

Medical diagnosis: iron deficiency anemia

- History of present illness: pt. complain from dizziness, shortness of breath, palpitation and fatigue, and the result of hematology

Past surgical history: ovarian cystectomy since 2007.

Obstetric history :

The pt. have menarche at age of 11 years, her menstruation is regular, with 28 days interval, duration of menstruation is 6 days, LMP:2/4/2011,
EDD: 9/1/2012.

Nursing diagnosis (1).
- Imbalanced nutrition less than body requirement related to inadequate intake of iron as evidence by hemoglobin level is 7.4 g/dl

Goal : promoting iron intake.
Objectives: at the end of my shift the pt will be able to define the important of adequate nutrition during pregnancy, and take the supplemental iron.

Intervention:
1) administer blood transfusion of blood cell as the doctor order and monitor the vital signs.
2) administer iron replacement as ordered.
3) Assess diet for inclusion of foods rich in iron that can be attained by the low socioeconomic states such as Beans, and soybeans, Green vegetables spinach , and enhance its absorption by vitamin c such as drink lemon, and postponed tea after 2 hours.

Evaluation: the goal is met as evidence by the pt define the benefit of adequate nutrition for her health, and take supplemental iron with lemon juice without caffeine.

Nursing diagnosis (2).

Ineffective peripheral tissue perfusion related to compromise circulation as evidence by pulse defect.

Goal : decrease in Venus congestion.
Objectives: at the end of my shift the edema of legs will decrease from 3+ to 1+ .

Iron is absorbed into the body and is stored in the liver as ferritin and is released to the bone marrow to make new red blood cells. Risk factors of Iron deficiency Anemia- Women who menstruate and when their periods are heavy, people with gastrointestinal disorders when iron cannot be absorbed into the body, people who undergo bariatric surgeries etc. are some of the high risk group of people who may have iron deficiency anemia. In the case study Ms. A has heavy menstrual bleeding for almost ten to twelve years and she is also takes high doses of aspirin during her menstrual flow days (Harper, 2012).
