DOI: 6/15/2008. The patient is a 63-year-old male rotary slot operator who sustained injury to his wrists due to repetitive motion of flipping corrugated sheets.
The patient was subsequently diagnosed with carpal tunnel syndrome, unspecified upper limb.

As per medical report dated 1/18/16, patient complains pain in his right elbow and bilateral hands and wrists. He has increased pain in bilateral upper extremities from elbow to hands worse on the right. He states uses Transcutaneous Electrical Nerve Stimulation unit and helping a lot with his pain decrease. He also has some pain relief with the use of his medication regimen. He rated pain as 6 with medications and as 8 if without medications. Patient also mentions of feeling depressed. Current medications include Norco, Theramine, Trepadone, Sentra, Cele/Lyrica/Lido rub, Tram/Bac Rub, and gabapentin.

Current request is for 30 Capsules of Effexor XR/Venlafaxine HCl 37.5 mg between 4/25/2016 and 6/24/2016; 90 Tablets of Neurontin/Gabapentin 600 mg between 4/25/2016 and 6/24/2016.

FDA approval is for treatment of depression and anxiety disorders. Off-label recommended treatments are for neuropathic pain, diabetic neuropathy, fibromyalgia, and headaches. It is an option in first-line treatment of neuropathic pain. The initial dose is generally 37.5 to 75 mg/day with a usual increase to a dose of 75 mg b.i.d or 150 mg/day of the ER formula. In like manner, Gabapentin (Neurontin®) is also recommended for or some neuropathic pain conditions and fibromyalgia. It has also been considered as a first-line treatment for neuropathic pain. She alternates gabapentin with Lyrica and takes tramadol with good relief and tolerates it well.
