I-on scale 1-10? Intensity of the pain.
Q- Quality of the pain. Sharp shooting-Dull aching. Do you have this pain all the time? Or does it come and go? How often do you get it? How long does it last?
O- onset of the pain.
R- Radiation – Does it stay or move around?
A- Any other associated symptoms like nausea, vomiting, diarreah, headache? Does anything make your pain better? Does anything makes your pain worse?
P-Past history

M-Medication
H-Hospitalization
U-Urinary symptoms Do you have any difficulty in peeing?
G-Do you have any gastro intestine symptoms ex. Constipation and diarrhea.
S-Surgery in the past.

O- Occupation- what do you do for living?
S-Social History-smoking , drinking, recreation drugs and certain diseases are related with particular
occupations.
S- Sexual History- Are you sexually active next are you gay, straight, bisexual. Do you use protection.
Familial diseases: Diseases that are common in a particular family like diabetes, hypertention
Hereditary/Genetic diseases: They pass from one generation to another through chromosomes or genes.
Screening Test
Papsmear- after 3 years of sexual intercourse.
Mammogram- MSG 30- 45 years between USG
PSA Test- Prostate Specific Antigen
Colonoscopy
Physical examinatioin: If patient sates if they have abdominal pain.
