Sunk Cost

People tend to have a strong persistence in what they have invested a lot of money or spent much time. In this case, the two leaders’ judgment on to continue climbing or to turn around should have been decided based on safety, not clients’ intention.
Consistency
Uniformity of attitude or account.
People tend to forget or deprecate the rule or agreement when the goal is close. In this case before the summit day, the leaders stressed the importance of turning back when falling behind schedule, but eventually they allowed their clients to go to the summit after 2 pm.
Bottle Neck
A single or small number of components that limit the performance or capacity of an entire system or resource.
In this case, the leaders allowed some clients who were not skilled at 8,000 meter peaks to join and continue climbing even after they were exhausted. These people caused delay because the guides and sherpas had to take care of them while they should have done other activities or preparation.
Preparation
The activity of putting or setting in order in advance.
Although the leaders recognized that there was no room for mistake, their preparation was not enough. There were some logistical problems that one of the leaders spent much time to resolve. This made him exhausted.
