This intake form takes into account the biological, the psychological, and the social aspect of the client. In fact, this form is so extensive it is asked that you fill it out prior to coming in for your first visit. As far as the biological aspect, the form targets the clients family medical history. It then feeds into the psychological aspects such as their current state of being/symptoms. One example the form gives is the clients suicide risk. Lastly it feeds into the social aspects such as substance abuse and what effects that had on your life. Overall, this form outlines the Multidimensional Framework.

I have noticed that parents tend to be overly protective and involved with their children’s information and medical history. Now that FERPA is in effect, we must be careful and ensure our clients confidentiality to the fullest. We must also realize that they have the right to not disclose certain information. I also have noticed that the form asks questions about substance abuse, and what specific drugs one has tried or do regularly. I am not sure if they have a Spanish version but having one is necessary in all populations because you do not know when a parent or youth has a language barrier.
e) Is there enough space to write what is needed in each answer space? Why or why not?
There is definitely an adequate amount of space to answer questions given in the intake. The form even allows you to add any additional information that they feel that should be disclosed that the intake did not cover or address. The amount of space given reflects each question individually. I also noticed that the space given was intended for the amount of information that needed to be disclosed. If a doctor needs you to go further into detail, those questions will be addressed in the initial meeting in order to gain a better understanding of the client and their history.
f) Should the intake be re-formatted so it is easier to read? Why?
The intake form is written relatively well and easy to read, I just think that the medication names still can be a little tricky and overwhelming for some.
